2025/26 Optional Insurance Application
Applications and payment for maintaining your previous years EHC and Dental level, dependent coverage renewal and dependent additions are due July 15, 2025. Please note that maintaining your previous year (24/25) benefit level applies to EHC and Dental only. Please contact us for payment plan options. A reminder that premiums paid to maintain a previous level and dependent premiums are eligible to be reimbursed from your health care spending account.

NEW Allstate Benefits Voluntary Critical Illness Insurance – Open Enrolment June 1 – June 30
The Member Benefit Trust is pleased to announce a limited-time opportunity for members to purchase Critical Illness Insurance through Allstate Benefits. This voluntary coverage is available for enrollment between June 1 and June 30, 2025, and offers guaranteed acceptance with no medical questions required. This coverage is in addition to existing coverage for which you may be eligible.
Key Features of the Coverage:
- Guaranteed Acceptance: No medical questions during the open enrollment period.
- Dependents Covered: Your dependent children are covered at no additional cost for 50% of your chosen benefit amount. You can also choose to add coverage for your spouse for an additional premium.
- Covered Illnesses: Includes conditions like heart attack, stroke, major organ failure, kidney failure, invasive cancer, Alzheimer’s disease, Parkinson’s disease, and more.
Benefit Amount Options:
- Plan 1 – $15,000
- Plan 2 – $30,000
- Plan 3 – $50,000
This coverage is available for yourself and your dependents, and it’s important to note that coverage is Guaranteed Issue (no medical questions). It is strictly on a voluntary basis and is paid directly by you to Allstate Benefits via PAD.
